Why Record Failures Against The Stylesheet

Accessibility failures in a theme are usually systematic rather than scattered. A single muted colour used across the theme fails once in the stylesheet and everywhere it is applied. Recording each failure against the token or component it comes from, rather than the page it appeared on, means one fix clears them together.
